Whole Person Integrated Care

Frank Porter Graham Child Development Institute is collaborating with The Carter Clinic PA to offer mental health and primary care services to 10 rural counties in the southeastern region of North Carolina as recipients of a grant from Eastpointe Managed Care Organization for Whole Person Integrated Care. As a continuous quality improvement initiative, the program aims to use a data-driven approach to improve complex case management for better mental and physical health outcomes. The clinical services will offer an innovation of direct-client payments to address barriers for appointment scheduling and attendance arising from pre-screened social determinants of health. This effort includes a prospective clinical study that focuses on client health outcomes, social determinants of health, and preferences for Whole Person Integrated Care (WPIC) as well as an evaluation of WPIC program implementation to identify potential cost-savings for the WPIC approach.
